ABSTRACT

ObjectiveTo obtain content characteristics of inorganic elements in Scutellariae Radix (aged 1-4 years), and to explore the feasibility of identifying the growth years of Scutellariae Radix based on characteristic spectrum of inorganic elements combined with chemometric models. MethodAfter microwave digestion, the contents of Mn, Zn, Ca, Fe, Mg, Na, K, Cr, Cu, Se, As, Cd, Hg, Pb and Ni in 21 batches of Scutellariae Radix were determined by inductively coupled plasma atomic emission spectrometry (ICP-OES) and inductively coupled plasma mass spectrometry (ICP-MS). Meanwhile, characteristic spectrum of inorganic elements in samples was drawn. The identification model was constructed to discriminate the growth years of Scutellariae Radix based on the combination of principal component analysis (PCA), Fisher discriminant function and support vector machine (SVM). ResultThe contents of Mn (7.79-36.48 μg·g-1), Zn (10.12-31.43 μg·g-1), Cu (6.38-17.20 μg·g-1), K (2.98-13.89 μg·g-1), Mg (3.45-7.78 μg·g-1) and Ca (2.32-7.09 μg·g-1) in Scutellariae Radix were detected by ICP-OES and ICP-MS, and their contents increased with the prolongation of growth years. PCA results showed that Cu, Ni, Cd, Na, Mg, Fe, Ca, Zn, Mn and Hg were characteristic elements of Scutellariae Radix. Samples with different years could be divided into four categories in the spatial characteristic diagram of Fisher discriminant analysis. The correct rate of SVM model for identifying the growth years of samples was 95.2%. ConclusionThis established method is accurate and rapid for discriminating the growth years of Scutellariae Radix, which can provide reference for the identification of other Chinese medicinal materials. It is suggested that some elements should be considered as indexes in subsequent construction of the quality evaluation system of Scutellariae Radix.

ABSTRACT

OBJECTIVE:To eva luate content constituent characteristics and antiviral activity of main flavonnoids in genuine and non-genuine Scutellaria baicalensis ,and to investigate quality-efficiency relationship and effective substance. METHODS : Totally 8 batches of S. baicalensis from different origins (S1-S8)were collected to prepare its water extract lyophilized powder. UPLC method was used to determine the contents of baicalin ,wogonoside,baicalein and wogonin ,and calculate their content constituent ratio. According to the content constituent ratio of flavonoids in S. baicalensis from different origins ,the mixture of the above four components was prepared as the corresponding simulated sample of flavonoids (E1-E8). Using ribavirin as positive control,MTT assay and CPE method were used to investigate half toxic concentration (TD50)of the water extract from 8 batches of S. baicalensis and their corresponding simulated samples to human laryngeal carcinoma cell Hep-2 and their half inhibition concentration(IC50)to respiratory syncytial virus (RSV);treatment indexes (TI)were calculated. Pearson correlation analysis for the content of baicalin ,wogonoside,baicalein and wogonin with their anti-RSV activity (IC50)was performed by SPSS 17.0 software. RESULTS :The contents of baicalin and wogonoside in sample S 4(from Hebei Chengde )were the highest ,and the contents of baicalein and wogonin in sample S 6(from Inner Mongolia- 2)were the highest. The contents of above 4 components were the lowest in sample S 6,S6,S7(from Beijing ),S4. The contents of flavonoid glycosides in sample S 4 were relatively higher, while those of corresponding glycosides were relatively lower ;the content constituent ratio of baicalin , wogonoside,baicalein and wogonin was 1∶0.224∶0.111∶0.013. The contents of flavonoid glycosides in sample S6 wererelatively lower ,while those of glycosides were relatively 126.com higher;the content constituent ratio of above 4 components was 1∶0.241∶0.713∶0.106. TC 50 of S. bai calensis water extracts from different origins to Hep- 2 cells was all higher than 50 μg/mL, IC50 to RSV was 11.11-51.74 μg/mL;TI was 1.86-5.20. TC 50 of corresponding simulated samples of flavonoids was 23.11-52.23 μg/mL,while IC 50 to RSV was 4.87-14.61 μg/mL;TI was 1.85-4.75. The anti-RSV effects of water extract of S 4 from genuine origins(Hebei Chengde )and its corresponding simulated sample E 4 were the strongest. Correlation analysis showed that the correlation coefficient between the contents of 4 flavonoids and antiviral activity was less than 0.5,and there was no significant correlation between them. CONCLUSIONS :When the content constituent ratio of 4 main flavonoids in S. baicalensis was 1∶0.224∶ 0.111 ∶ 0.013,it has a strong inhibition effect on RSV. S. baicalensis from genuine origins has better constituent characteristics of flavonoids,which may be the important material basis for the best antiviral effect of genuine medicinal material.

ABSTRACT

Objective To study the effect and mechanism of dexmedetomidine combined with sufentanil on postoperative analgesia in elderly patients with abdominal surgery. Methods Ninety-six elderly patients having undergone abdominal surgery in the First Affiliated Hospital of Wenzhou Medical University from January 2016 to June 2017 were enrolled, and they were divided into a control group and an observation group by random number table method, 48 cases in each group. General anesthesia was performed in the operation, and after surgery venous analgesic pump was used for analgesia in both groups. Analgesic method: the control group was given sufentanil 2 μg/kg and tropisetron 5 mg; the observation group was given dexmedetomidine 2 μg/kg, sufentanil 2 μg/kg and tropisetron 5 mg. The changes of pain and sedation score, conventional indexes [systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), respiratory rate (RR), pulse blood oxygen saturation (SpO2)], oxidative damage indexes [lipid peroxide (LPO), glutathione peroxidase (GSH-Px), Cu-Zn superoxide dismutase (Cu-Zn SOD)], stress response indexes [cortisol (Cor), epinephrine, norepinephrine (NE)], platelet activation indexes granule membrane protein-140 (GMP-140), thromboxane A2(TXA2) and the incidence of adverse reactions were observed in both groups. Results ① After surgery the visual simulation score (VAS) and Ramsay score in both groups were higher than those before surgery, and showed a tendency firstly increased and then decreased, and reached to peak value 2 hours after operation [VAS score:the control group was 3.24±0.98 vs. 1.95±0.93, observation group was 3.19±1.03 vs. 1.98±0.95; Ramsay score:the control group was 3.26±0.51 vs. 1.90±0.45, observation group was 3.77±0.53 vs. 1.92±0.42], began to decline 6 hours after operation, reached to valley value 48 hours after operation, and there was no significant difference in VAS scores between the two groups (2.02±0.64 vs. 1.98±0.95), Ramsay score was significantly higher in observation group than that in control group (2.59±0.41 vs. 2.10±0.21). ② Since 2 hours after the operation, the SBP, DBP, HR and RR in the observation group began to be lower than those in control group, 12 hours after surgery their values reached their valleys [SBP (mmHg, 1 mm Hg = 0.133 kPa): 113.2±13.5 vs. 122.1±10.3, DBP (mmHg): 67.5±9.9 vs. 76.4±8.6, HR (bpm): 64.5±6.9 vs. 71.4±7.5, RR (bpm): 14.8±1.1 vs. 15.8±0.8, all P < 0.05]; while SpO2did not change a great deal. ③ LPO, Cor, epinephrine, NE, GMP-140, TXA2of two groups after operation were higher than those before operation, GSH-Px and Cu-Zn SOD were lower than those before operation. However, LPO, Cor, epinephrine, NE, GMP-140, TXA2in observation group were significantly lower than those in control group [LPO (μmol/L): 6.4±0.8 vs. 9.8±1.1, Cor (ng/L): 148.2±19.1 vs. 239.3±27.8, epinephrine (μg/L): 124.2±13.9 vs. 207.1±23.5, NE (μg/L): 109.2±14.8 vs. 183.3±21.6, GMP-140 (μg/L): 27.13±3.82 vs. 39.06±4.83, TXA2(ng/L): 422.30±53.74 vs. 610.43±73.21, all P < 0.05], GSH-Px and Cu-Zn SOD in observation group were significantly higher than those in the control group [GSH-Px (U/L): 426.7±58.7 vs. 307.9±51.2, Cu-ZnSOD (μg/L): 311.3±42.5 vs. 231.6±34.1, all P < 0.05].④ The incidence of adverse reaction nausea in the observation group was significantly lower than that in the control group [4.17% (2/48) vs. 37.5% (18/48)]. Conclusion Dexmedetomidine combined with sufentanil used in elderly patients after abdominal surgery has significant analgesic effect, can effectively inhibit platelet activation, and decrease the contents of GMP-140 and TXA2.

ABSTRACT

OBJECTIVE:To investigate the analgesia effect of low-dose ketamine combined with sufentanil after radical sur-gery of esophageal cancer. METHODS:Totally 153 patients of pre-implementation esophageal cancer surgery were randomly divid-ed into group A,B and C. After induction of anesthesia in patients in each group,patients in group A administered sufentanil 2 μg/kg;group B were treated with sufentanil 2.5 μg/kg;group C were treated with sufentanil 2 μg/kg + ketamine 85 μg/(kg·h). Visual analog(VAS)score and Ramsay score before and after anesthesia 4,8,16,24 and 48 h,analgesia pump pressing times and inci-dence of adverse reactions in each group were observed. RESULTS:Ramsay score in 4,8 and 16 h in each group were significant-ly higher than before anesthesia,the differences were statistically significant(P<0.05),however,compared with before,there was no significant difference in the Ramsay score in 48 h among each group(P<0.05);VAS scores in each group(except for group C in 48 h) at each time point were significantly higher than before,group C

ABSTRACT

OBJECTIVE:To compare mental state and drug safety of puerperant underwent propofol and etomidate painless abortion after regain of consciousness. METHODS:102 puerperant underwent painless abortion were selected and randomly divided into control group and trial groups with 51 cases in each group. The control group received Propofol injection 2 mg/kg intravenous-ly,and the trial group received Etomidate fat emulsion injection 0.2 mg/kg intravenously. The changes of patient’s mental state were evaluated with Positive and Negative Emotions Scale (PANAS) before anesthesia and after regain of consciousness. The changes of HR,BP,MAP and SpO2 were observed and compared between 2 groups before anesthesia and 1,2,3 and 5 min after anesthesia. The anesthetic effect and ADR were compared between 2 groups. RESULTS:Compared to before anesthesia,positive mood scores of the trial group were increased slightly after regain of consciousness,while that of the control group were increased significantly;there was statistical significance before anesthesia and after regain of consciousness (t=9.023,P0.05). The total effective rate of anesthesia in control group was 80.4%,while that of trial group was 78.4%;there was no statistical signifi-cance between 2 groups (χ2=1.273,P>0.05). The incidence of nausea,vomiting,dizziness and other ADR were similar in 2 groups,but the incidence of fantasy(39.2%)in control group was significantly higher in trial group(11.8%);there was statistical significance between 2 groups(χ2=10.120,P<0.05). CONCLUSIONS:Propofol and etomidate have good stability on the respira-tory and circulatory system,anesthetic effects are correspond to the incidence of ADR. Etomidate has a better safety. But the psy-chological status of patients in the propofol group is preceding than etomidate group.
